You can request a name, address, or phone number change online using the form below. You may also use this form to request replacement cards and keytags. Replacement cards or keytags they will be mailed to you within 3 days. You may also request changes at the Atlantic Customer Service Desk. If you would like to use your card for check cashing, stop by the service desk and provide a valid photo ID. Please provide us with your e-mail address so that we may contact you if we have any questions regarding your request. Thank you.
